New patient visit

If you are coming in for the first time:

• Please complete the New Patient forms and bring them with you to the visit.
• Please bring your insurance card each time you come to the office.
• If you have a co-payment, it will be collected when you check in at each visit.

Well Child Appointment

• Well child visits can be scheduled up to 3 months in advance.
• You can request an appointment by registering for access to the patient portal (instructions are on the home page) or by calling (510)-848-2566
• Please consult the Well Child Visit Schedule for appropriate Appointments

Sick Visit

• Please call in advance to set up appointments.
• Children are seen by appointment only. (This way, we can allow plenty of time for each patient.)
• We are happy to provide same day appointments for urgent medical problems.
• Be sure to make separate appointment times for each sibling who is to be seen.

Prenatal Interview

Our doctors do prenatal consultations.  During this visit prospective families can get to know their doctor, and ask any questions they many have.   This is also a chance for the doctors to familiarize the family with the practice in general and their style and beliefs in particular.
